>>ALL INFORMATION IS REQUIRED to be filled out on our liability waiver in order to be acceptable for each participant to jump.
>>If a person is UNDER 18 years old, he/she must bring in a liability waiver signed his/her PARENT or his/her FULL-TIME LEGAL GUARDIAN in order to jump.
>>If
a person is 18 or OLDER, he/she must bring a VALID PHOTO ID with a BIRTH-DATE to sign for his/her own liability waiver in order to jump.
>>The liability waiver can be FAXED to our office at 925-682-5868, or it can be SCANNED and EMAILED to coninfo@jumpskyhigh.com, or signed in our facility itself.
